PLEASE READ PRIOR TO ALLOWING YOU CHILD(REN) TO PARTICIPATE
As the parent and/or guardian, I acknowledge that my child(ren) and I are present as guests to participate in activities at this location. I am aware that activities (such as jumping, climbing, and sliding) can be hazardous if not supervised. By allowing my child(ren) to participate, I acknowledge that I have read and will adhere to the posted list of cautions and further agree to abide by any rules or regulation and to follow any guidelines or instructions that the event coordinator/operator may provide orally or in writing. I hereby assume all risks of injury and damage to persons or property. I further release, indemnify and hold harmless the Special Event Fun, Inc. from and against any and all liabilities, claims, demands, costs, expense or compensation of whatever nature in connection with any loss, damage or injuries to person or property caused or sustained by me and my child(ren).
I assume all risks and responsibilities for the safety and well-being of such minor child(ren) and agree by allowing my child(ren) to participate on the Rockwall/Climbing wall that I am entering into this Liability Waiver and Indemnity on behalf of such minor child(ren). I have read the Liability Waiver and Indemnity statements above and the Rules/Regulations and Cautions and agree to be bound by/adhere to them.
By allowing my child(ren) to participate in the Rock Wall/Climbing Wall I agree to the liability and waiver document.
I understand that all parents or guardians are responsible for our children and their activities.
I understand that NO unattended or unsupervised children will be allowed.
